IGF-1 is a 70 amino acid straight chain polypeptide. It has been shown to be identical to Somatomedin-C. The molecule shows approximately 50% sequence homology with proinsulin and demonstrates a number of properties similar to insulin. It is believed to mediate the growth promoting effects of human growth hormone and its level in plasma is highly dependent on growth hormone levels. IGF-1 is reduced in states which produce GH deficiency and elevated in GH excess (acromegaly and pituitary gigantism). It is polypeptide produced mainly in the liver which stimulates growth and differentiation in a wide range of different tissues including nascent cartilage. Its rate of secretion depends on somatotropin levels and diet. Its blood concentration varies with age and sex. This test is conducted in parallel with the somatotropin assay in children with retarded growth, in pituitary insufficiency or in acromegaly. It is useful for postoperative monitoring in acromegaly, for monitoring GH therapy for retarded growth and in the diagnosis of Laron dwarfism.
