Calcium is required for the proper functioning of muscle contraction, nerve conduction, hormone release, blood coagulation, and formation of bone. It is primarily regulated by the actions of the parathyroid hormone, vitamin D, and calcitonin. Half of the calcium in the blood is free and is metabolically active, while the remaining half is bound to albumin and is metabolically inactive. The total calcium test measures both the free and bound forms. HIGH total calcium (hypercalcemia) is associated with hyperparathyroidism, cancer, hyperthyroidism, sarcoidosis, tuberculosis, prolonged immobilization, excess vitamin D intake, or kidney transplant. LOW calcium level (hypocalcemia) is associated with hypoparathyroidism, extreme deficiency in dietary calcium, decreased levels of vitamin D, magnesium deficiency, increased levels of phosphorus, pancreatitis, renal failure, or malnutrition
